Complete the rollover enrollment form and return it to us. We'll help you choose a long-term investment mix. Transfer your money to the Plan. Give your employer the following information: - The name of the plan trustee: U.S. Bank N.A. - The name of the IRS qualified plan: IBEW Local Unions Savings and Security Plan - Your account number: This is your Social Security number. When you give your employer this information, they will send a check to you (in some cases, the employer may send the check directly to Scarborough). You should forward it to Scarborough for deposit to your account. We suggest sending checks via overnight mail with a receipt; we will reimburse you for the mailing costs. For Payroll Deductions... Download an application form or call us at 800-223-7608 to request an enrollment form. Complete the enrollment form and return it to us. We'll tell you how to enroll with your employer, and we'll help you choose a long-term investment mix. For IRA Rollovers... Download an application form or call us at 800-223-7608 to request an IRA rollover form and a rollover enrollment form. Complete both forms and return them to us, along with a copy of your most recent account statement, if available. We'll forward the IRA rollover form to your current IRA custodian. Your current IRA custodian will forward a check to Scarborough for deposit into your account. We will contact you upon receipt and deposit into your account according to your instructions. (Some institutions may forward the check directly to you, in which case you would then forward to Scarborough).
